ID card to verify the name of your network.

necessarily updated daily; therefore, before selecting a
participating doctor or hospital shown in the online
directory, be sure to verify with the network that your
particular doctor or hospital is still in the network. You
can do this by calling the 800 number listed.
The networks are solely
responsible for the content and accuracy of their web sites
to which you will be linked.

For a complete listing of benefits,
exclusions and limitation, please refer to the plan document. In
the event there are discrepancies with the information given here, the
terms and conditions of the coverage documents will govern.
